All new cases in Dallas District Courts must include a Cover Sheet to ensure that the Original Petition is filed correctly.  The Cover Sheet should be included as the last page of the filed document or as an attachment.

NOTE:  The Cover Sheet should not be confused with a cover letter.  A cover letter is placed in front of unsigned orders and other documents that are filed but do not require a file stamp.
